Student Activity Fee
Please complete the following form and remit with student activity fee payment. Only one form per family is necessary, list the names of all Arcola and MHS students and submit this form along with payment. Payment may be made in person at student orientation or back to school night, mailed, or sent in with your student prior to September 21, 2010. If mailed or submitted with student, please send with a self-addressed stamped envelope so that a receipt may be issued.
